Your Contact Request
Disability United provides information about SSDI, SSI, and VA disability benefits. You can read the guides without sending a request. The request form asks for your name, a phone number or email address, ZIP code, and a few program-specific selections. Do not include Social Security numbers, medical documents, full dates of birth, or detailed medical histories.
Your entries stay in the page’s memory until you select the contact disclosure and choose “Send My Help Request.” That action sends your contact details and selected answers to Disability United’s SecureFormPro CRM. The record includes the disclosure you accepted, its version, the time of acceptance, the page address, and limited campaign information when present. A confirmation number means the request was stored; it does not mean an application or appeal was filed, a representative accepted your case, or benefits were approved.
How Information Is Used
The information is provided to review and handle your request and to contact you using the details you supply. SecureFormPro stores the request as a service provider. This website does not automatically send marketing texts, emails, or calls when you submit. A request does not create an attorney-client or other representation relationship.
Test Mode and Unsent Information
When a form displays “Test mode,” submitted requests are recorded as tests in SecureFormPro and do not trigger a call or message. Unsent entries remain in this page’s memory and clear when you refresh or use “Clear Details & Restart.” Clearing the page does not delete a request already stored in the CRM. Do not send real personal details through a test form.
Requests About Children
For a child’s claim, provide your own contact details as the parent or guardian. Do not send a child’s contact details or medical records through this form.
Technical Information and Cookies
Hosting and CRM infrastructure process request information to operate and secure these services. The form uses a short-lived security cookie to help prevent unauthorized submissions. It contains no contact details. This website adds no advertising pixels or analytics cookies. It may retain campaign labels and the landing-page path in page memory and include them with a request. Contact details and answers are not written to browser storage. Home-screen support caches a generic offline page, not your request or preparation plan.
External Websites
SSA, VA, and other linked websites have their own privacy practices. A phone link opens your device’s calling application. Contact the responsible agency directly when you need to file an application, check a claim, or meet a deadline.
Terms of Use
This website provides general educational information, not legal advice or an eligibility decision. Disability United is independent of and not affiliated with the Social Security Administration or the U.S. Department of Veterans Affairs. Those agencies decide claims. Information and requirements can change; review the official sources and your own notices. This website does not preserve appeal deadlines or guarantee benefits, representation, or a response time.
Privacy Contact and Retention
This deployment is under review. The legal operator, privacy-request contact, retention schedule, and any additional authorized recipients must be confirmed before this site accepts public production requests. Test records are used only to verify the integration and are identified separately from ordinary leads.
