To apply for Social Security Disability Insurance (SSDI) benefits there is the main application, known as form SSA 16 BK, as well as three other forms that need to be filled out. These forms are available on the Social Security website or your local Social Security Administration (SSA) office:
- Main application for SSD (SSA-16-BK)
- A disability report for providing your medical condition (SSA-3368-BK)
- Work history report (SSA-3369-BK)
- Medical records release (SSA-827-BK)
When filling out the forms, be as truthful and honest as possible. Make sure you give full details of your condition, such as the medication you take, the side effects they may be causing and how this hinders you from making a living. You will also need to fill in details about the last job(s) you were engaged in such as the type of work performed. This is to determine whether any of your skills are transferrable and whether you could find gainful employment doing any other job. SSDI examiners will go through your application thoroughly to judge if you fit eligibility criteria.
