Name the Coverage You Actually Have
List VA health care separately from Medicare. For Medicare, note whether you use Original Medicare or a Medicare Advantage plan, and identify any separate prescription drug plan. Original Medicare includes Part A and Part B. Medicare Advantage is a private plan alternative that includes those benefits and usually drug coverage; network and prior-authorization rules may apply.
Use the current cards and notices to identify plan names and coverage dates. A VA disability compensation notice is not a substitute for this health coverage list. This worksheet is for arranging planned, non-emergency care, not deciding whether to enroll in or drop coverage. Urgent and emergency care have separate rules.
Start With the Place and Service
Write down the provider, the actual care location and the service being discussed. A community clinic may see patients under more than one coverage arrangement. Its ability to accept Medicare does not establish that your appointment is VA-authorized community care.
VA and Medicare generally cannot pay for the same item or service. VA does not bill Medicare, and Medicare does not pay for care at a VA facility. If VA authorizes only some services during a non-VA hospital stay, Medicare may cover other services that meet Medicare’s rules. That possibility is not a promise that Medicare will pay any balance left over.
Ask which coverage will be used for this particular service and what remains unconfirmed. Tell the provider about your other coverage rather than expecting the office to infer it from a single card.
Give Each Question to the Right Contact
Keep a contact for each responsibility. The person who books an appointment may not be the person who can confirm a referral, explain plan coverage or correct a bill. Ask for the appropriate team when a question is outside the first person’s role.
- VA health care team or community care office: Is this planned non-VA care approved, and what provider, service and period does the authorization cover?
- Provider’s scheduling or billing office: Which coverage is recorded for this visit, and is there an unresolved referral or billing question?
- Medicare Advantage plan, if you have one: Is this provider in the relevant network, and does this service need plan approval?
- Medicare: Where can I get an explanation of Original Medicare coverage or a Medicare notice? Use the official Contact Medicare page linked below.
- VA pharmacy or Medicare drug plan: Which prescription, pharmacy and coverage arrangement are we checking? Keep this separate from the appointment question.
- State Health Insurance Assistance Program, or SHIP: Can a counselor help me understand my Medicare coverage and choices? Medicare lists SHIP as a source of free counseling.
Two Fictional Appointments, Different Questions
These examples are invented to show contact planning, not to predict a coverage decision. Daniel uses VA health care and Original Medicare. His VA team refers him to a community specialist. He asks the VA contact whether the referral is ready and which care it includes; he asks the specialist’s office whether it has the matching information. He does not treat his Medicare card as proof of VA approval.
VA’s community care instructions say the authorization letter identifies the approved provider, care and period of approval. Daniel keeps the letter with his appointment record. If the specialist proposes an additional service, he asks whether it is included and who must resolve that question instead of assuming the original referral covers everything.
Renee uses VA health care and a Medicare Advantage plan. She is considering a separate appointment at a non-VA clinic through that plan. She contacts the plan about the specific clinic and service, then checks what coverage the clinic has recorded. A neighbor’s statement that the clinic takes Medicare does not answer her plan-network or approval questions.
Renee also tells her VA care team about outside care so the clinicians can coordinate information. That communication serves a different purpose from a coverage confirmation: keeping a clinician informed does not itself authorize a bill.
Check Prescriptions Separately
A completed medical visit does not answer every pharmacy question. Record the prescription, prescriber and intended pharmacy, then contact the VA pharmacy or Medicare drug plan for the route you expect to use. You cannot use VA drug benefits and Medicare drug coverage for the same drug at the same time.
VA’s non-VA pharmacy guidance has specific rules for eligible urgent-care prescriptions and distinguishes them from regular, non-urgent prescriptions. Do not assume every retail pharmacy can fill every prescription under VA coverage. Ask about the exact medication and pharmacy before relying on that arrangement.
If a coverage question could interrupt an existing prescription, tell the pharmacist and prescribing clinician what is unresolved. This contact worksheet does not advise changing or stopping medication. Keep a current medication list available to your care teams.
Copy This Coverage Contact Card
Use one card for each planned appointment or service. Keep full insurance numbers, medical records and authorization documents in a secure place; the version shared for family logistics can simply say where those records are stored. This page does not collect the information.
- Care being arranged: Provider, location, service and proposed appointment date.
- Expected coverage: VA-authorized care, Original Medicare or the exact Medicare Advantage plan; mark it unconfirmed until checked.
- VA contact: Care team or community care office, contact method and the question it needs to answer.
- Medicare contact: Medicare or plan member services, contact method and the specific coverage question.
- Provider contact: Scheduling or billing team and the coverage currently recorded for the visit.
- Pharmacy question: Prescription, intended pharmacy and the responsible pharmacy or drug-plan contact.
- Evidence of the answer: Date, office or staff role, reference number if provided, and where the written confirmation is stored.
- Next action: Unresolved item, person following up and agreed follow-up date.
Finish With Confirmed Answers
Review the card before the appointment. Replace a vague note such as insurance handled with the actual answer: which office confirmed which service, under which coverage, and what still needs attention. A message left or a scheduled appointment is not the same as a coverage decision.
If a later notice or bill conflicts with the record, contact the office responsible for that notice and keep the documents together. For decisions about changing Medicare coverage, use Medicare, your plan or a SHIP counselor rather than drawing an enrollment conclusion from one appointment. Disability United provides educational navigation; the responsible program or plan determines coverage.
